I understand that by giving permission to the Tionesta Community Association, I do hereby relinquish all rights to the photograph(s). Copyright and ownership belongs to Tionesta Community Association to publish or utilize the photograph(s) as they see fit.

Liability Waiver:

I, the undersigned, being aware of my own health and physical condition, and having knowledge that my participation in any event that may be injurious to my health, am voluntarily participating in a physical activity.
